TKB Registration Information

REGISTRATION IS CLOSED.

Church Affiliation​

There are three churches (Tallowood, Chapelwood, and MDPC) that register players for Tallowood Kingdom Basketball. Each church has a separate website and registration system. Accordingly, all players on a team must register through the same church.

 

Importantly, a registered player desiring to change church affiliation will need to withdraw (at a penalty, per the refund policy) and then re-register through one of the other churches.

 

If you have any doubt about church affiliation, please confirm in advance that all preferred teammates plan to register through the same church.

 

Registration Process

  • The registration link provided below is for Tallowood teams only.

  • A fee of $193 per player will be charged via credit card.

  • Only a parent or legal guardian can register a child for participation in Tallowood Kingdom Basketball.

  • The registration process includes the signing of both a Waiver and Release of Liability and a Code of Conduct statement. Please review both statements (shown below) before registering.

  • We recommend using a personal computer or phone, rather than an office device, to access the registration system.

  • PRE-K players must be 4 years old to be registered. For inquiries regarding younger Pre-K players, TKB Executive Director approval is required.

 

Code of Conduct Statement

Players, coaches, parents, and visitors will conduct themselves at all times in a manner consistent with the mission statement of Tallowood Kingdom Basketball:

To provide witness of Jesus Christ by offering children an opportunity to learn and play basketball with their friends in a Christian sports ministry environment.

Specifically:

  • All adults and children will practice good sportsmanship,

  • Players will wear appropriate basketball attire during games, including their team jersey,

  • Inappropriate language will not be used,

  • Referees and scorekeepers will not be verbally abused or otherwise disparaged,

  • Coaches and parents will strive to create and maintain a positive, uplifting environment,

  • Parents will ensure their children (players) appropriately understand these expectations.

 

Waiver and Release of Liability Statement

 

NOTE: THIS FORM MUST BE READ AND SIGNED BEFORE THE PARTICIPANT IS PERMITTED TO TAKE PART IN ANY EVENT, INCLUDING PRACTICE OR TOURNAMENT. BY SIGNING THIS AGREEMENT, THE PARTICIPANT AND PARENT/GUARDIAN, IF APPLICABLE, AFFIRMS HAVING READ IT.

 

IN CONSIDERATION of being allowed to participate in any way in Tallowood Kingdom Basketball, I, the undersigned, acknowledge, understand, and agree that:

 

  1. I RISK BODILY INJURY, INCLUDING PARALYSIS, DISMEMBERMENT, DISABILITY AND DEATH, and while particular rules of the sport, skills, equipment, and personal discipline may reduce this risk, THIS RISK OF SERIOUS INJURY DOES EXIST, AS WELL AS THE RISK OF DAMAGE TO OR LOSS OF PROPERTY; and,
     

  2. I KNOWINGLY AND FREELY ASSUME ANY AND ALL SUCH RISKS OF INJURY AND DISEASE, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERS, and assume full responsibility for my participation; and,
     

  3. I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe any unusually significant hazard during my presence or participation, or if I observe any concern in my readiness for participation, I will immediately bring such to the attention of the nearest official and remove myself from participation; and,
     

  4. I, FOR MYSELF, AND ON BEHALF OF MY HEIRS, ASSIGNS, PERSONAL REPRESENTATIVES, AND NEXT OF KIN, HEREBY RELEASE, INDEMNIFY, HOLD HARMLESS AND PROMISE NOT TO SUE TALLOWOOD BAPTIST AND/OR TALLOWOOD KINGDOM BASKETBALL, ITS MANAGERS, OWNERS, OFFICERS, OFFICIALS, VOLUNTEERS, EMPLOYEES, AGENTS, AND/OR OTHER PARTICIPANTS, SPONSORS, ADVERTISERS, AND, IF APPLICABLE, THE OWNERS AND LESSORS OF PREMISES USED FOR THE ACTIVITY ("RELEASEES"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, AND/OR LOSS OR DAMAGE TO PERSON OR PROPERTY, WHETHER CAUSED BY THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, EXCEPT THAT WHICH IS THE RESULT OF GROSS NEGLIGENCE OR WANTON MISCONDUCT, TO THE FULLEST EXTENT PERMITTED BY LAW.
     

I have read this Release of Liability and Waiver Agreement, fully understand its terms, understand that I have given up substantial rights by signing it, and sign it freely and voluntarily without any inducement.

 

FOR PARENTS/GUARDIANS OF PARTICIPANTS OF MINORITY AGE

(UNDER AGE 18 (or under such other age of consent as may be applicable in any given jurisdiction) AT TIME OF REGISTRATION)
 

This is to certify that I/we, as parent(s)/guardian(s) with legal responsibility for this participant, do consent and agree not only to the release above regarding this participant’s participation in any Tallowood Kingdom Basketball team or event, but also for myself/ourselves, and my/our heirs, assigns and next of kin, to release and indemnify the Releasees from any and all Liability incident to my/our minor child’s involvement as stated above, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES, to the fullest extent permitted by law. As parent(s)/legal guardian(s) for this participant, I/we also hereby give to Tallowood Kingdom Basketball, the right to use the name, image and likeness (without any right of approval) of this participant for any use to promote Tallowood Kingdom Basketball programs and teams.

 

AUTHORIZATION - USE OF LIKENESS

Participants, or their legal guardian(s), in Tallowood Kingdom Basketball events agree to be filmed and photographed by photographers authorized by Tallowood Kingdom Basketball or the legal guardian(s) hereby give to Tallowood Kingdom Basketball the right to use their name, picture, and likeness (without any right of approval) for any use to promote the event in which they participate as well as to promote future events and activities of Tallowood Kingdom Basketball. The duration of this authorization shall commence with the Participant’s registration with Tallowood Kingdom Basketball for any team or event and continue before, during and for sixty (60) months after the conclusion of the Participant’s participation in Tallowood Kingdom Basketball event or team.

REFUND POLICY

Players may be withdrawn and registration fees refunded upon request as follows:

  • September 15 - November 15
    Refund amount: $143
    .00

  • November 16 - December 15
    Refund amount: $108.00

  • No refunds will be issued after
    December 15

Late Registration will open
December 4
 if space is available.

 

INTERRUPTED SEASON REFUND POLICY

1. In the event the season is interrupted, cancelled, or disrupted due to no fault of Tallowood Kingdom Basketball, the ministry will refund 75% of registration fees (Expenses incurred in preparation for the season include jerseys, medals, facility rental commitments, program staff expenses, payment processing fees, background checks) on a prorated basis.

2. In the event that rental gyms are closed or unavailable due to no fault of Tallowood Kingdom Basketball, the ministry will work towards a resolution. However, in the event one or more courts are not available, the registrant is aware the program may be disrupted or games cancelled.
