As a participant in a position within the league, you will need to supply the following information:
 

• Social Security Number

• Drivers or Identification Card Number

• Banking Account Information (routing and account number)

  for direct deposit​

• Complete online Ministry Safe Training

 

​Employment Information

You will be asked to complete a W-9 form (fillable PDF), save it to your desktop, and then upload it during the registration process. Also, at the end of the registration process, there will be a link for you to complete a direct deposit form. You will need your banking account information (found on a blank check) readily available. These forms will then be sent directly to the Tallowood Baptist Church business office.

 

Background Check and Sexual Abuse Awareness Training

A current Background Check (renewed every 2 years) is required of all paid Staff, 18 years of age or older.

 

Additionally, to help ensure the safety and security of all our players, TKB and Tallowood Baptist Church have partnered with MinistrySafe to provide sexual abuse awareness training for administrative staff, referees, and scorekeepers aged 18 and over. This training equips recipients to understand better the facts and misconceptions related to child sexual abuse, including the basic characteristics of an abuser, the grooming process of the offender, and key indicators of child sexual abuse.

 

This online MinistrySafe training (renewed every 5 years) can be completed at your convenience but must be done prior to Dec 9, 2023. The training includes a video that should take approximately 60 minutes to complete, followed by a 25-question online quiz. The video portion does not have to be completed all at once; however, once you begin the quiz, answers will not be saved until it is fully complete.

 

A Background Check (if needed this year) and MinistrySafe training (evidence of prior training accepted) will be initiated at the end of the registration process.

 

​Referee and Scorekeeper Training

Mandatory training for Referees and Scorekeepers will be provided at Tallowood Baptist Church on the dates and times shown below. Those registrants will be asked to indicate which ONE of these two available sessions they plan to attend.
